Tiivistelmä
Tämä pro gradu -tutkielma muodostuu kahdesta eri osasta. Ensimmäinen osa on tieteellinen artikkeli: "Äitien imetyksestä selviytyminen ja imetystuen saanti neuvolasta lapsen ollessa kuuden viikon ikäinen" (Pehkonen S, Aho AL, Hannula L & Kaunonen M), joka on hyväksytty julkaistavaksi Hoitotiede -lehteen. Tutkielman toinen osa on yhteenveto-osa: "Äitien taustatekijöiden yhteys imetyksestä selviytymiseen ja kokemus terveydenhoitan imetysohjeista lapsen ollessa kuuden viikon ikäinen".
Tieteellisessä artikkelissa tarkoituksena oli kuvata äitien imetyksestä selviytymistä ja neuvolan terveydenhoitajalta saatua imetystukea lapsen ollessa kuuden viikon ikäinen. Lisäksi tarkoituksena oli selvittää imetystuen määrän ja imetyksestä selviytymisen välistä yhteyttä. Yhteenveto-osassa tarkoituksena oli kuvata äitien taustatekijöiden yhteyttä imetyksestä selviytymiseen ja äitien kokemusta terveydenhoitajan imetysohjeista lapsen ollessa kuuden viikon ikäinen. Tutkimusaineisto kerättiin strukturoidulla kyselylomakkeella eräissä eteläsuomalaisissa sairaaloissa synnyttäneiltä äideiltä (n=421) vauvan ollessa kuuden viikon ikäinen. Tutkimusaineisto analysoitiin SPSS -tilasto-ohjelmaa apuna käyttäen.
Suurin osa äideistä (85 %) koki selviytyvänsä imetyksestä yleisesti joko hyvin tai erittäin hyvin. Parhaiten äidit kokivat osaavansa auttaa vauvan rinnalle hyvään imemisotteeseen ja tulkita vauvan viestejä imemisvalmiudesta. Imetysongelmia ilmeni 50 % äideistä, joista vain 35 % koki saaneensa ohjausta riittävästi tai melko riittävästi. Keskimäärin eniten äidit kokivat tarvitsevansa emotionaalista tukea, jota he myös parhaiten saivat. Hoitajalta saatu myönteinen palaute imetyksestä ja uskonvahvistus imetyksestä selviytymiseen jatkossakin arvioitiin parhaiten toteutuneiksi. Imetystuen määrän ja imetyksestä selviytymisen välillä oli heikko positiivinen lineaarinen riippuvuus, joka oli tilastollisesti merkitsevä. Mitä enemmän imetystukea tarvittaessa saatiin, sitä paremmin imetyksestä selviydyttiin.
Uudelleensynnyttäjät ja aiemmin imettäneet äidit selvisivät imetyksestä tilastollisesti merkitsevästi paremmin kuin ensisynnyttäjät. Myös äidit, joiden aiempi imetyskokemus oli ollut erittäin myönteinen, selvisivät imetyksestä tilastollisesti merkitsevästi paremmin kuin äidit, joiden imetyskokemus oli ollut kielteisempi. Äitien taustatekijöistä iällä, siviilisäädyllä, työssäkäynnillä tai koulutuksella ei ollut tilastollisesti merkitsevää yhteyttä imetyksestä selviytymiseen. Äidit kokivat terveydenhoitajan ohjeet keskimäärin parhaiten kannustaviksi, ymmärretäviksi ja yhdenmukaisiksi aikaisempiin tietoihin nähden. Sen sijaan äidit kokivat ohjeet keskimäärin heikoiten molemmat puolisot huomioiviksi, yksilöllisiksi ja tarkoiksi.
Tutkimuksien tavoitteena oli tuottaa tietoa, jota voidaan hyödyntää äitien imetyksen tukemisessa sekä imetysohjauksen sisällön ja opetuksen kehittämisessä.
ABSTRACT
Mothers´ coping with breastfeeding and the guidance in breastfeeding when the baby is six weeks old
This master´s thesis consists of two sections. The first section is the research article: "Mothers' self-reported success with breastfeeding and the breastfeeding support when the baby is six weeks old" (Pehkonen S, Aho AL, Hannula L & Kaunonen M), which has been accepted to be published in the Journal of Nursing Science. The second section is called: "Mothers´ coping with breastfeeding and the guidance in breastfeeding when the baby is six weeks old".
The purpose of the research article was to describe mothers' ability to cope with breastfeeding and the breastfeeding support given by the public health nurse when the baby is six weeks old. In addition, the association between the amount of breastfeeding support and mothers' success with breastfeeding was studied. The purpose of this study was to describe the connection of mothers´ background information to their coping with breastfeeding. In addition, mothers´ experience of the guidance in breastfeeding given by the public health nurse was studied. The data were collected with a questionnaire from mothers (n=421) who had delivered a baby in some hospitals in South Finland, when the baby was six weeks old. The data was analyzed statistically.
Most mothers (85 %) reported they were coping well or very well with breastfeeding. Best they felt they could help the baby in a good feeding position and to read the baby s readiness to nurse. As much as 50 % of mothers had had troubles with breastfeeding. Only 35 % of those mothers reported they were given enough or nearly enough guidance. Mothers both received, and majority of mothers also needed emotional support most. In mothers' experience receiving positive feedback and support to cope with breastfeeding were evaluated having been succeeded best in guidance. There was a weak positive linear correlation between the amount of breastfeeding support and mothers' success with breastfeeding and it was statistically significant. The more mothers received breastfeeding support, the better they succeeded with breastfeeding.
Mothers who had given birth and breastfed before succeeded better with breastfeeding than the first-time-mothers. Also mothers, who had had a very positive breastfeeding experience earlier, succeeded better with breastfeeding than mothers who had had a more negative breastfeeding experience. The differences were statistically significant. Mothers´ age, marital status, working or education did not have a statistically significant connection with breastfeeding. Mothers felt that the best things in guidance were that it was supportive, understandable and consistent with the earlier knowledge. Instead, mothers felt that the weakest things in guidance were individuality, accuracy and taking both parents into account.
The aims of these studies were to produce information that can be used to support mothers in breastfeeding and to improve guidance and education in breastfeeding.
