Tiivistelmä
Tämä pro gradu -tutkielma koostuu kahdesta osasta. Ensimmäinen osa on Hoitotiede-lehden teemanumeroon 4/2014 hyväksytty tieteellinen artikkeli "Iäkkäiden päivystyspotilaiden uusintakäyntien syyt ja niihin yhteydessä olevat tekijät" (Ylä-Mononen S, Palonen M, Koivisto A-M, Åstedt-Kurki P, 2014). Tutkimuksen tarkoituksena oli kuvata yli 75-vuotiaiden päivystyspotilaiden aikaisten uusintakäyntien syitä ja niihin yhteydessä olevia tekijöitä. Tutkimuksessa haettiin vastausta tutkimuskysymyksiin: kuinka paljon yli 75-vuotiailla potilailla on uusintakäyntejä päivystyspoliklinikalla ja mitkä tekijät ovat yhteydessä näihin käynteihin? Kyseessä oli rekisteritutkimus, ja tutkimusaineisto kerättiin yhden yliopistosairaalan päivystyspoliklinikan perusterveydenhuollon potilasrekisteristä (N=4611). Aineisto analysoitiin SPSS for Windows 20.0 -ohjelmalla. Päivystyspoliklinikalle saapuminen ja päivystyspoliklinikalta kotiuttaminen aamuyön tunteina lisäävät iäkkään potilaan uusintakäynnin riskiä. Potilailla, jotka saavat päivystyspoliklinikan hoidon tarpeen kiireellisyysluokituksen ESI3, on suurempi riski palata uusintakäynnille kuin alemman kiireellisyysluokituksen saaneilla potilailla. Tulodiagnooseista vatsa- ja lantiokipu, kuume ja verenvuoto hengitysteistä ennustavat uusintakäyntiä. Tutkimustulosten perusteella on mahdollista tunnistaa iäkkäät potilaat, joilla on suurin riski uusintakäyneihin päivystyspoliklinikalle. Potilaiden hoitoa ja hoitotyötä voidaan kehittää niin, että uusintakäynnit vähenevät.
Toisena osana on rekisteritutkimuksen menetelmäkuvausta, jossa esimerkkinä käytetään "Iäkkäiden päivystyspotilaiden uusintakäyntien syyt ja niihin yhteydessä olevat tekijät" -tutkimusta. Rekisteritutkimuksella on Suomessa pitkät perinteet. Rekistereihin kerättyä aineistoa voidaan käyttää terveydenhuollon laadun parantamiseen, toiminnan suunnitteluun ja erilaisten tieteellisten tutkimuksien tekemiseen. Kun rekisteritietoja käytetään yhtenä tai ainoana tieteellisen tutkimuksen aineistona, puhutaan rekisteritutkimuksesta. Rekisterit voidaan henkilötietolain perusteella jakaa neljään eri rekisterityyppiin; toimintayksikkökohtaisiin potilasrekistereihin, valtakunnallisiin terveydenhuollon henkilörekistereihin, tutkimusrekistereihin ja ns. erillisrekistereihin. Henkilötietolaki säätelee rekisteritutkimuksen tekoa. Rekisteritutkimuksissa tutkimusaineiston hankkiminen on nopeaa ja edullista perinteisiin aineistonkeruumenetelmiin nähden. Tämä säästää rajallisia tutkimusresursseja. Viranomaisten ylläpitämiin rekistereihin on kerätty vähän tietoa hoitotyön ilmiöistä. Hoitotyön kirjaaminen tapahtuu useimmiten manuaalisesti tai sähköisesti niin, että tieto on ei-rakenteellisessa muodossa. Hoitotyön kirjaaminen on siirtymässä kokonaan sähköiseen muotoon osaksi sähköistä potilaskertomusta. Tämä mahdollistaa tulevaisuudessa entistä kattavampien rekisteritutkimusten tekemisen hoitotyön ilmiöistä.
ABSTRACT
This master's thesis consists of two parts. The first part is a scientific article. The article, "The Causes and Correlates of Emergency Department Re-admissions in Older Adults" (Ylä-Mononen S, Palonen M, Koivisto A-M, Åstedt-Kurki P 2014) has been accepted for the themed issue of Hoitotiede-journal 4/2014. The objective of the study was to characterize the causes and possible correlates of early emergency department (ED) re-admissions (≤72 h) of adult patients aged over 75 years. The study questions were: how much do over 75-year-old patients experience early emergency department (ED) re-admissions and what are the causes for these visits?
The sample was collected retrospectively from a hospital patient registry (N=4611). The material was analyzed using the statistical software, SPSS, for Windows 20.0. ED admission and discharge in the early hours increased the risk of re-admission. Patients with an ESI level of 3 had an increased risk for early re-admission compared to the patients with lower ESI levels. As causes of ED visits, abdominal and pelvic pain, fever, and hemoptysis were predictive of early re-admission. It is possible to identify older adults with an elevated risk of early re-admission. The patients' care and nursing can be developed in order to decrease the re-admissions.
The purpose of the second part is to describe the register study as a research method, using the study "The Causes and Correlates of Emergency Department Re-admissions in Older Adults" as an example. The register study has a long tradition in Finland. The material collected for registers can be used for improving the quality of public health services, for planning their operation and as the material for different scientific studies. The register study is a study in which register information is the only material used for the scientific study. Registers can be divided into four different types on the basis of the Personal Data Act (22.4.1999/523): functional unit-specific registers of patients; the national person registers of the public health service; study registers; and so-called special registers. The Personal Data Act directs the creation of the register study. It is quick and advantageous to obtain research material in contrast to traditional data collection methods; this saves limited study resources. A small amount of information has been gathered only about the phenomena of nursing in relation to the registers maintained by the authorities. Nursing information is usually recorded manually or electronically, so the information is in an unstructured form. Recording of nursing is partly undertaken using electronic patient data, which is moving to a totally electronic form, and this will make undertaking register studies particularly covering the phenomena of nursing possible in the future.
