Tiivistelmä
Tämän tutkimuksen tarkoituksena oli kuvata vaativaa hoitoa ja pitkäaikaisseurantaa tarvitsevien lasten hoitoon liittyviä tarpeita ja niiden tärkeyttä vanhempien näkökulmasta. Tarkoituksena oli myös kuvata, miten moniammatillinen hoitotiimi vastaa vanhempien tarpeisiin. Tutkimuksen tavoitteena oli saada tietoa, miten tällä hetkellä erikoissairaanhoidossa moniammatillinen tiimi vastaa vaativaa hoitoa ja pitkäaikaisseurantaa tarvitsevien lasten vanhempien lapsen hoitoon liittyviin tarpeisiin.
Tutkimus toteutettiin poikittaistutkimuksena erään yliopistollisen sairaalan alle 18-vuotiaita syöpään ja diabetekseen sairastuneita, sekä alle 1500 g painoisena syntyneitä lapsia hoitavissa yksiköissä. Tutkimusaineisto kerättiin postitse lähetetyllä kyselylomakkeella. Kyselyyn vastasi vanhempi. Perheiden (n=244) yhteystiedot saatiin potilasrekistereistä. Uusintakyselyä ei tehty. Lopullinen tutkimusaineisto oli 98 vastauksesta ja vastausprosentti 40,2. Kyselylomake muodostui kolmeosaisesta Family Inventory of Needs-Paediatric II (FIN-PED II) -mittarista, jolla mitattiin vanhempien tärkeänä pitämiä tarpeita, niihin vastaamista ja lisätiedon tarvetta näistä tarpeista, sekä taustakysymyksistä ja avoimesta kysymyksestä. Aineisto analysoitiin tilastollisin menetelmin sekä sisällön analyysillä. Tilastollisen merkitsevyyden rajana pidettiin p<0,05.
Vastaajilla oli keskimäärin yli 15 tarvetta ja ne arvioitiin erittäin tärkeiksi. Tärkeimpiä tarpeita olivat tuntea, että henkilökunta huolehtii vilpittömästi lapsestani, saada tietoa lapseni voinnin muutoksista, tuntea että oli toivoa, tietää mitä hoitoja lapseni oli saamassa, tietää että voisin esittää kysymyksiä milloin tahansa ja saada tietoa ja perusteluja termeillä, jotka olivat minulle ymmärrettäviä. Noin 85 % vastaajista oli sitä mieltä, että moniammatillinen hoitotiimi oli vastannut hyvin tai erittäin näihin tärkeimpiin tarpeisiin. Mitä huonommin vanhemmat kokivat hoitotiimin vastanneen heidän tärkeinä pitämiin tarpeisiinsa, sitä enemmän heillä oli tarpeita, joista olisivat tarvinneet lisää tietoa. Lisätietoa tarvittiin myös sitä enemmän, mitä huonommin hoitotiimi oli tarpeisiin vastannut. Vastaajan iällä ja koulutustasolla, oli yhteyttä tarpeiden lukumäärään. Vastaajan koulutustaso oli yhteydessä tarpeiden tärkeyteen. Vanhempien työstä poissaolo, lapsen diagnoosi ja lapsen sairaalassaoloaika olivat yhteydessä hoitotiimin kokoon. Mitä pienempi hoitotiimi lapsella oli, sitä tärkeämpinä vanhemmat pitivät tarpeitaan. Avoimeen kysymykseen vastanneilla vanhemmilla oli myös tiedon saantiin, vertaistukeen, arkeen ja lapsen kanssa selviämiseen, sekä kuuntelemiseen ja kohtaamiseen liittyviä tarpeita moniammatilliselta tiimiltä.
Johtopäätöksenä voidaan todeta, että moniammatillinen hoitotiimi vastaa keskimäärin hyvin erilaisten perheiden yksilöllisiin tarpeisiin. Vanhemmat tarvitsevat tukea sairastuneen lapsen sisarusten huomiointiin sekä perheen arjesta selviämiseen.

English abstract:
The purpose of this study was to describe the parental needs and how important those needs are in families of children with intensive and long term care. The purpose was also to describe how these needs are met by multidisciplinary team. The aim was to get information about how multidisciplinary care teams meet needs of the parents of children with intensive and long term care at this moment in special health care settings.
This cross-sectional study was conducted in the university hospital units of children with cancer, diabetes and babies who were born under 1500 g of weight. The data was collected by postal questionnaires from one parent of the family with under 18-year-old child. The contact details of families (n=244) were found in the register of patients. 99 parents took part in this study. One questionnaire was rejected. 98 parents completed Family Inventory of Needs-Paediatric II (FIN-PEDII), demographic questions and one open-ended question. FIN-PED II-inventory measured importance of care needs, how these needs were met by multidisciplinary team from the parents` point of view, and parents` need for further information. The data was analysed statistically and with content analysis. Significance level was set at p<0,05.
The respondents on average over 15 needs and they were considered as very important. The most important care needs were to feel that the healthcare professionals were sincere in caring about my child, to be informed of changes to my child`s condition, to feel there was hope, to know what treatment my child was receiving, to know I could ask questions any time and to have explanations given in terms that were understandable to me. About 85 % of the respondents considered that these needs were met well or completely by multidisciplinary team. The worse parents needs were met by multidisciplinary team, the more information about their needs they needed. Also the worse parents needs were met, the more needs they have for further information. Age and educational level were related to the number of the needs considered as important. The respondents` educational level were related to the importance of the needs. Parents absence from work, child`s diagnosis and the time of hospitalization were related to the size of the multidisciplinary team. The smaller the team was, the more important parents were considered their needs. According to responses to open-ended questions, parents have also needs for receiving information, peer support, coping in everyday life, getting heard and respected by multidisciplinary team.
It can be concluded that the tailored needs from different families were approximately well met by the multidisciplinary team. Parents need more support how they can take their child`s siblings into account and managing their everyday life.
