<title>ESHRE guideline: ovarian stimulation for IVF/ICSI: an update in 2025</title>

STUDY QUESTION: What is the recommended management of ovarian stimulation, based on the best available evidence in the literature?SUMMARY ANSWER: This updated ESHRE guideline on ovarian stimulation for IVF/ICSI provides 121 recommendations, answering 21 key questions on ovarian stimulation for IVF/ICSI.WHAT IS KNOWN ALREADY?: Before the ESHRE guideline on ovarian stimulation for IVF/ICSI was published in 2019, ovarian stimulation for IVF/ICSI had only been discussed briefly in the National Institute for Health and Care Excellence guideline on fertility problems and in a statement by the Royal Australian and New Zealand College of Obstetricians and Gynaecologists.STUDY DESIGN, SIZE, DURATION: The guideline was developed according to the structured methodology for ESHRE guidelines. The 18 key questions from the 2019 version of the guideline were revised by the Guideline Development Group (GDG). This resulted in the addition of one new key question, the splitting of the key question on fertility preservation in three separate key questions (fertility preservation for women facing gonadotoxic treatment, elective oocyte cryopreservation, and oocyte donation) and several new interventions being added to the existing key questions. Papers published between 31 October 2018 and 2 February 2025 and written in English were included. The critical outcomes for this guideline were efficacy in terms of cumulative live birth rate per started cycle or live birth rate per started cycle, as well as safety in terms of the rate of occurrence of moderate and/or severe ovarian hyperstimulation syndrome (OHSS).PARTICIPANTS/MATERIALS, SETTING, METHODS: Based on the available evidence, recommendations were formulated and discussed until consensus was reached within the GDG. Following stakeholder review of the initial draft, the final version was approved by the GDG and ultimately by the ESHRE Executive Committee.MAIN RESULTS AND THE ROLE OF CHANCE: The guideline provides a total of 121 recommendations: 42 recommendations remained unchanged in 2019, 4 recommendations were reworded for better understanding, 29 recommendations were updated in view of new evidence, and 46 new recommendations for 2025 have been formulated. The guideline provides 4 recommendations on pre-stimulation evaluation, 7 recommendations on pre-treatment therapies, 50 recommendations on pituitary suppression and ovarian stimulation, 17 recommendations on monitoring, 18 recommendations on triggering of final oocyte maturation and luteal support, and 8 recommendations on the prevention of OHSS. In addition, the guideline provides 17 recommendations on fertility preservation, both oncologic and elective, and oocyte donation. These include 90 evidence-based recommendations, of which only 42 were formulated as strong recommendations and 48 as conditional, as well as 29 good practice points and 2 research-only recommendations. Of the evidence-based recommendations, none were supported by high-quality evidence, 6 by moderate-quality evidence, 36 by low-quality evidence, and 148 by very low-quality evidence. To support future research on ovarian stimulation for IVF/ICSI, a list of research recommendations was provided.LIMITATIONS, REASONS FOR CAUTION: Several newer interventions are not well studied yet. For most of these interventions, a recommendation against the intervention or a research-only recommendation was formulated based on insufficient evidence. Future studies may require these recommendations to be revised.WIDER IMPLICATIONS OF THE FINDINGS: The guideline provides clinicians with clear advice on best practice in ovarian stimulation, based on the best evidence available. The spouses of women with gestational diabetes mellitus (GDM) seem to be at risk of developing type 2 diabetes and cardiovascular diseases (CVDs). We comparatively analysed the risk factors, lifestyle, socioeconomic factors, and health of the spouses of women with (cases, n = 599) and without (controls, n = 586) GDM. This cross-sectional study utilized data from the Finnish Gestational Diabetes study. Data of the spouses were collected using a structured questionnaire: socioeconomic factors, smoking, alcohol consumption, health, own perinatal health, and family history of diabetes and CVDs. Age-adjusted odds ratios (aORs) were analysed using multivariate logistic regression. The mean ages of the cases and the controls were 33.5 years and 31.2 years, respectively [mean difference: 2.4 years, 95% confidence interval (CI): 1.68-3.02]. The mean body mass index of the cases (26.9 kg/m2) was 0.78 kg/m2 (95% CI: 0.34-1.21) higher than that of the controls. Fewer cases attained the highest educational level (13.5% vs. 16.9%, aOR 0.64, 95% CI: 0.46-0.90). The cases reported more often alcohol consumption (85.4% vs. 78.7%, aOR 1.59, 95% CI: 1.16-2.17); chronic disease, impairment, or disability (17.4% vs. 12.6%, aOR 1.43, 95% CI: 1.03-2.00); or mental disorder (8.0% vs. 5.1%, aOR 1.63, 95% CI: 1.01-2.64). The cases reported more risk factors for adverse health outcomes, more chronic diseases and mental disorders than the controls. Therefore, lifestyle counselling should also be provided to the spouses of women with GDM.

Aim: This study investigated frequent attenders (FAs) in primary health care in Helsinki, Finland, focusing on their service use, sociodemographic characteristics (sex, age, and language), and diagnostic profiles using registry data. Methods: Register-based cohort data were drawn from administrative records in primary, specialised, and oral health care of the City of Helsinki and Helsinki University Hospital (2015–2019; n = 297 845). FAs were defined as the top decile of annual primary health care users. Physician and nurse face-to-face visits were included. Patients were categorised by how many years (1–5) they met FA criteria. Statistical analyses were performed using the χ2 test or, when appropriate, Poisson regression with robust variance estimation to estimate prevalence ratios (PRs). Results: Frequent attenders had more than seven annual face-to-face primary health care visits. Although representing only 15.9% of patients, FAs accounted for nearly half of all health care visits. FAs were more often women, aged over 65, and native Finnish speakers. Chronic diseases and multimorbidity were more prevalent among FAs. Chronic skin wounds were strongly associated with frequent attendance. As attendance persisted, visit distribution shifted: physician visits increased from 0.7 among non-FAs to 6.4 among 5-year FAs, while nurse visits rose from 0.6 to 11.4 annually, comprising nearly two-thirds of contacts among persistent FAs. Conclusion: FAs represent a small but high-need group in primary health care. Their service use is driven not only by physicians but disproportionately by nurses, highlighting the importance of including nursing care in research and resource planning.

The self-controlled case series (SCCS) method is frequently employed to explore the relationship between transient exposures and subsequent health events, utilizing data from individuals who have experienced the event of interest. Conventional spline-based SCCS models typically do not account for overlapping exposure periods and fail to accommodate complex interactive effects among multiple exposures. In this paper, we introduce a novel semiparametric SCCS method that employs a functional partial-linear single index (PLSI) link function, allowing for the estimation of overlapping exposure risks. Our approach offers greater interpretability and flexibility compared with existing methods by consolidating multiple exposures into a single index and modeling complex interactions through a nonparametric link function. We validate our model through simulation studies comparing its performance with standard methods under various practical exposure settings. Furthermore, we apply our method to two real-world datasets involving MMR vaccination and malaria chemoprevention, demonstrating its practical utility and enhanced capability to handle multiple, overlapping exposures effectively. Our findings suggest that the PLSI-SCCS model is a robust tool for modern epidemiological and pharmaceutical research, providing a nuanced understanding of exposure effects, particularly in complex multi-exposure scenarios.
