Tiivistelmä
Nuoruuden valinnat ja terveyskäyttäytyminen määrittävät aikuisuuden käyttäytymismalleja ja terveydentilaa, minkä vuoksi nuoret ovat tärkeä terveyden edistämisen kohderyhmä. Nuoruusiässä psykoseksuaalisen kehityksen myötä lapsen seksuaalisuus muuttuu kohti aikuisuuden seksuaalisuutta, ja samalla kiinnostus päihteitä kohtaan herää. Seksuaali- ja päihdekasvatuksen on havaittu edistävän nuorten tietoja, taitoja ja asenteita ja tätä kautta nuorten hyvinvointia ja terveyttä. Seksuaali- ja päihdekasvatuksen toteutus kouluissa eroaa kuitenkin alueellisesti ja koulukohtaisesti, mikä asettaa nuoret eriarvoiseen asemaan. Tamperelaisen Seksuaali- ja päihdeseikkailupäivän eli Sepäse-päivän tavoitteena on tarjota kaikille 8.-luokkalaisille sama tieto seksuaaliterveydestä ja päihteistä. Tämän tutkielman tutkimustehtävänä oli selvittää nuorten seksuaaliterveyden ja päihteiden tietoja, taitoja ja asenteita sekä sitä, onko Sepäse-päivään osallistuneiden ja osallistumattomien nuorten tai sukupuolten välillä eroja.
Tutkielma toteutettiin kvantitatiivisena survey-tutkimuksena. Tutkimuskohteena olivat yhdeksäsluokkalaiset nuoret yhdeksästä tamperelaisesta yläkoulusta, joihin Sepäse-päivä oli suunniteltu järjestettäväksi keväällä 2020, mutta COVID-19-pandemian vuoksi vain neljä koulua osallistui päivään. Viidestä Sepäse-päivään osallistumattomasta koulusta kolme kieltäytyi tutkimukseen osallistumisesta. Tutkielman aineistonkeruu toteutettiin Sepäse-päivän pohjalta laaditulla kyselylomakkeella, ja aineisto kerättiin tamperelaisilta 9.-luokkalaisilta lokakuun 2020 ja tammikuun 2021 välillä. Saatu aineisto (n = 373) analysoitiin IBM SPSS Statistics 26 -ohjelmistolla. Aineiston kuvailussa käytettiin frekvenssi- ja prosenttijakaumia. Aineiston analyysiin käytettiin χ2-testiä, Fisherin tai Monte Carlon testiä sekä Mann Whitneyn ja Kruskall Wallisin testejä.
Tämä tutkielma osoitti, että nuorten seksuaaliterveyden tiedoissa on parantamisen varaa ja reilu viidesosa nuorista kaipasi siitä lisätietoja. Sepäse-päivään osallistuneiden nuorten tiedot seksuaaliterveydestä olivat paikoin Sepäse-päivään osallistumattomia paremmat. Seksuaaliterveyden tiedot olivat tytöillä poikia ja muunsukupuolisia paremmat. Nuorten seksuaaliterveyden taidot havaittiin hyviksi ja asenteet pääasiassa myönteisiksi, eikä näissä ollut merkitseviä eroja ryhmien välillä. Sukupuolten välillä sen sijaan havaittiin selkeitä eroja sekä taidoissa että asenteissa.
Tutkielma osoitti, että myös nuorten päihdetiedoissa on parantamisen varaa ja 15 % nuorista kaipasi lisää tietoa niistä. Sepäse-päivään osallistuneilla nuorilla, tytöillä ja muunsukupuolisilla havaittiin paremmat päihdetiedot kuin päivään osallistumattomilla nuorilla ja pojilla. Erityisesti seksuaaliterveyden ja päihteiden yhteydestä muodostetut kysymykset tiedettiin paremmin Sepäse-päivään osallistuneiden joukossa. Valtaosa nuorista suhtautui päihteisiin varauksella, mutta huomattava osa salli ikäisensä nuoren päihteiden käytön, ja kannabista kohtaan etenkin poikien ja muunsukupuolisten asenteet olivat paikoin myönteiset.
Seksuaalisuuden ja sukupuolen moninaisuudesta nuorten tiedot havaittiin melko hyviksi ja Sepäse-päivään osallistumattomilla paremmiksi. Nuorten asenteet olivat pääasiassa myönteisiä, mutta niissä havaittiin jonkin verran suvaitsemattomuutta muttei merkittäviä eroja ryhmien välillä. Poikien tiedot olivat tyttöjä ja muunsukupuolisia heikommat ja asenteet selvästi suvaitsemattomampia. Nuorten tiedot, taidot ja asenteet, jotka koskivat turvataitoja ja omia rajoja, olivat hyviä ja myönteisiä. Näissä ei ollut ryhmien välillä suuria eroja, mutta poikien asenteet erosivat jälleen tytöistä ja muunsukupuolisista.
Tutkielma osoittaa nuorten seksuaali- ja päihdetiedon tarpeen, jonka vuoksi seksuaali- ja päihdekasvatusta kouluissa ja kodeissa on lisättävä. Sepäse-päivä voi olla käyttökelpoinen interventio vastaamaan nuorten tietotarpeisiin sekä keino edistää yhdenvertaista ja kaikki nuoret huomioivaa opetusta. The choices and health behavior in youth defines the behavior patterns and health in adulthood, which makes adolescents an important target group for health promotion. Following the psychosexual development in adolescence, a child’s sexuality shifts toward adult’s sexuality. At the same time, interest in substance use arises. Sexual and substance abuse education has been found to promote adolescents’ knowledge, skills and attitudes, and thus, their well-being and health. However, the implementation of sexual and substance abuse education in schools differ regionally and between schools, which puts adolescents in an unequal position. The goal of Tampere's Sexual and Substance Abuse Adventure day “Sepäse day” is to provide all 8th graders the same information about sexual health and drugs. The purpose of this thesis was to examine the knowledge, skills and attitudes of adolescents about sexual health and intoxicants and assess whether there are differences between Sepäse day participants and non-participants or between genders.

The study population consisted of ninth graders from nine upper secondary schools in Tampere, for which Sepäse day was supposed to be arranged in spring 2020. Due to the COVID-19 pandemic, only four schools participated in the day. Of the five schools not participating, three refused to take part in the research of the thesis. The data collection for this thesis was conducted using a questionnaire based on the information from Sepäse day, and it was collected from ninth graders between October 2020 and January 2021. The obtained data (n = 373) was analysed using IBM SPSS Statistics 26 software. Frequency and percentage distributions were used to describe the data. χ2-test, Fisher’s or Monte Carlo’s test, and Mann Whitney’s or Kruskall Wallis’ tests were used for data analysis.
The research demonstrated that there is room for improvement in adolescents’ sexual health knowledge, and over a fifth of the participants would like more information about the topic. The level of knowledge about sexual health was slightly more comprehensive among Sepäse day participants compared to non-participants, as well as with girls over boys and non-binary youths. Adolescents’ skills concerning sexual health were good, and the attitudes mainly positive. There were no notable differences between the groups. On the other hand, there were significant differences between the genders in both skills and attitudes concerning sexual health.
The research showed that there is also room for improvement in adolescents’ substance abuse knowledge, and 15 % of the subjects reported a need for more information about the topic. The adolescents who participated in Sepäse day, girls and non-binary youths had better knowledge of intoxicants than non-participants and boys. In particular, those who attended the Sepäse day showed better levels of knowledge in questions related to the relationship between sexual health and intoxicants. Most adolescents were cautious about drugs, but a notable proportion approved their peers’ usage of drugs and boys’ and non-binary youths’ attitudes towards cannabis were somewhat positive.
Adolescents’ apprehension regarding the diversity of sexuality and gender was found to be fairly good, and Sepäse day non-participants demonstrated increased levels of knowledge. Adolescents’ attitudes were mainly positive, yet some intolerance was observed. There were no significant differences between the groups. Boys showed weaker knowledge than girls’ and non-binary youths’, and their attitudes were more intolerant. Adolescents’ knowledge, skills and attitudes regarding sexual security skills and one’s own boundaries were good and positive. There were no major differences between the groups, but boys’ attitudes again differed from those of girls’ and non-binary youths’.
This thesis shows an increased need for sexual and substance abuse knowledge among adolescents. Therefore, the amount of sexuality and substance abuse education needs to be increased both in schools and homes. Sepäse day could act as a useful intervention method to meet the adolescents’ needs and promote equal and inclusive education for all adolescents.
