Tiivistelmä
Tämän pro gradu-tutkimuksen aiheena on vanhuspalvelulain läpivieminen Pohjois-Satakunnan peruspalvelu-liikelaitoskuntayhtymässä, joka tuottaa sosiaali- ja terveyspalveluita noin 21 500 asukaspohjalle. Kyseiset palvelut tuotetaan tilaaja-tuottaja -mallilla ja palveluiden tuottamisessa noudatetaan elämänkaarimallia. Ikäihmisten määrä on kyseisellä alueella huomattavan suuri. Vanhuspalvelulaki on vielä melko uusi lainsäädännöltään ja sen vuoksi sen läpiviemisen tutkiminen on ajankohtaista ja mielenkiintoista.
Tutkimuksessa tarkastellaan vanhuspalvelulakia osana muutosjohtamista ja ikäihmisten palvelurakennemuutosta. Tutkimuksessa avataan vanhuspalvelulakia ja ikäihmisten palveluihin suunnattua laatusuositusta ikääntymisen turvaamiseksi ja palvelujen parantamiseksi, joka osaltaan täydentää vanhuspalvelulakia. Näiden keskeisten muutosten kautta pyritään samaan kokonaiskuva siitä, miten Pohjois-Satakunnan peruspalveluliikelaitos kuntayhtymässä on huomioitu ja toteutettu vanhuspalvelulain tuomat velvoitteet.
Tutkimuksessa käytetään kvalitatiivista lähestymistapaa. Tutkimuksen aineisto koostuu yhteensä 13 henkilön haastattelusta, jonka kohteena on strateginen johto ja vanhusneuvostojen puheenjohtajat. Strategiseen johtoon kuuluu seitsemän haasteltavaa henkilöä, jotka edustavat luottamushenkilöitä ja strategisessa johdossa työskenteleviä viranhaltijoita. Tutkimuksessa haastatellaan lisäksi kuutta jäsenkunnan vanhusneuvoston puheenjohtajaa, koska halutaan saada selville myös kuntalaisten ääni , jota he edustavat. Analyysimenetelmänä on sisällönanalyysi. Analyysissä aineistosta muodostetaan kolme päälukua, joissa tuodaan esille haastateltavien näkemyksiä vanhuspalvelulain tuomien keskeisten muutosten toteutumisesta, muutosjohtamisesta ja yhteistyön toteutumisesta.
Tutkimuksen tuloksena voidaan todeta, että vanhuspalvelulain tuomat velvoitteet on huomioitu melko hyvin. Esimerkiksi ikäihmisten hyvinvointisuunnitelma on laadittu, henkilöstölle on järjestetty kuntoutuksen täydennyskoulusta ja palvelurakennetta on suunnattu avohoitopainotteiseksi. Suurin osa haastateltavista kokee, että muutosjohtamisessa on onnistuttu kohtuullisen hyvin. Huolta aiheuttaa jäsenkuntien taloudellinen tilanne, joka asettaa raamit osaltaan myös muutosjohtamiseen. Voidaan todeta, ettei yhteistyö eri sidosryhmien ja vanhusneuvostojen kanssa ole ollut riittävää. Vanhusneuvostoja ei ole otettu tarpeeksi mukaan suunnittelutyöhän. Tulevaisuutta ajatellen Pohjois-Satakunnan peruspalvelu-liikelaitos- kuntayhtymässä tulee panostaa vielä entistä enemmän erilaisiin yhteistyötapoihin, gerontologiseen sosiaalityöhön ja -kuntoutukseen sekä mahdollisesti raja-aitojen purkamiseen eri hallintokuntien väliltä.

This master's thesis deals with the process of putting the Act on Care Services for the Elderly into effect within PoSa, the business-based joint municipal authority for basic public services in North Satakunta region. PoSa provides services for about 21,500 population. The services are produced using the purchaser - provider model and public - private partnership. The elderly population of the region is remarkably large. The Act on Care Services for the Elderly is still rather new, so its realization is a current and interesting matter.
I study the Act on Care Services for the Elderly as a part of change management and changing service structure within care services for the elderly. I take a review of the Act on Care Services for the Elderly and the assisting Quality recommendation ("Quality recommendation to guarantee a good quality of life and improved services for older persons"). In this way I strive after a general view of how PoSa has reckoned and implemented the obligations of the Act on Care Services for the Elderly.
I employ a qualitative approach. The research material consists of interviews of 13 persons. 7 interviewees belong to the strategic management as elected representatives or leading officials, and 6 interviewees are chairpersons of local councils of the elderly, in order to bring up the voice of the citizens. The material has been analysed using content analysis. The analysed material is compiled into three main chapters, presenting the views of the interviewees on the realization of central reformations due to the Act on Care Services for the Elderly, on management of change, and implementation of cooperation.
It is possible to state that the obligations of the Act on Care Services for the Elderly have been taken quite well into consideration. For example, a wellbeing programme has been worked out, continuing education in rehabilitation has been arranged for the employees, and the service structure has been modified to favour outpatient services. Most interviewees think the management of change has been rather successful. They are worried about the economic situation of the member municipalities, which also sets limits for change management. It may be stated that there has not been sufficient cooperation with interest groups and the councils of the elderly. The councils of the elderly have not been adequately involved in the planning process. In the future PoSa has to invest increasingly in different ways to cooperate, gerontological social work and rehabilitation, and possibly breaking down barriers between the administrative branches.
