Tiivistelmä
Tutkimuksen tarkoituksena oli kuvata perhekeskeisyyttä aikuisen potilaan hoitotyössä ja perhekeskeisen hoitotyön oppimista vastavalmistuneiden sairaanhoitajien kuvaamana. Tutkimusaineisto kerättiin yksilöhaastatteluilla. Tiedonantajina oli 13 perusterveydenhuollossa tai erikoissairaanhoidossa aikuispotilaiden hoitotyössä työskentelevää sairaanhoitajaa, joiden valmistumisesta oli kulunut aikaa 12-15 kuukautta. Aineisto analysoitiin induktiivisella sisällönanalyysillä.
Vastavalmistuneet sairaanhoitajat kuvasivat perhekeskeisyyttä työssään painolastina, tiedon vaihtamisena, perheen sisäisenä tukena ja perhelähtöisenä yhteistyösuhteena. Perhekeskeisyys kuvattiin painolastina kun se liittyi sopivaan tilanteeseen tai aikaan, perheen kohtaamisiin liittyi kuormittavia hankaluuksia, puuttuva perhe kuormitti sairaanhoitajaa tai perhe jäi etäiseksi. Tiedon vaihtaminen perheen ja sairaanhoitajan välillä toteutui perheen tiedolla tukemisena tai perheen toimiessa tiedonantajana. Sairaanhoitajat kuvasivat velvollisuudekseen tiedottaa perhettä. Perheen sisäinen tuki sisälsi potilaskeskeisyyden, jossa perhe ajateltiin potilaalle ainutkertaisena ja perheen tuki potilaalle tärkeänä. Perheenjäsenten odotukset ohjasivat vuorovaikutusta. Perhelähtöisessä yhteistyösuhteessa korostui sairaanhoitajan vastuu perheestä hoidon kohteena. Sairaanhoitaja oli aloitteentekijä perheiden suuntaan ja koki vastuuta perheen hyvinvoinnista. Perhe nähtiin päivittäisenä osana hoitotyötä, jossa perheenjäsenten empaattinen tukeminen on keskeistä. Sairaanhoitajan ja perheen yhteistyötä kuvasi yhteinen toiminta.
Vastavalmistuneet sairaanhoitajat kuvasivat oppineensa hoitotyön perhekeskeisyyttä koulutuksessa, ihmisinä ja työelämässä. Koulutuksessa hoitotyön perhekeskeisyyttä opittiin teoriatiedon, opetussuunnitelman tai työharjoittelun mahdollistamana. Yksittäisillä opettajilla kuvattiin olleen merkitystä oppimiseen. Koulutus tuotti merkityksen hoitotyön perhekeskeisyydelle. Sairaanhoitajat kuvasivat oppimista ihmisenä yksilöllisten ominaisuuksien tai kokemusten mahdollistamana. Perheenjäsenten kohtaamiskokemukset sekä oman toiminnan arviointi tukivat oppimista. Perhekeskeisyyden oppiminen työssä kuvattiin tapahtuvan työpaikan toimintatapojen, taitojen karttumisen ja tiedon syventymisen myötä sekä toisilta sairaanhoitajilta oppimisena.
Tutkimuksen tulokset lisäävät tietoa perhekeskeisyydestä aikuisen potilaan hoitotyössä. Tulosten mukaan yhteistyösuhde sairaanhoitajan ja aikuisen potilaan perheen välillä ei ole itsestäänselvyys. Toisaalta vastavalmistunut sairaanhoitaja voi kyetä koko perhettä tukevaan yhteistyösuhteeseen. Tuloksia voidaan hyödyntää perhekeskeisyyden ja perhekeskeisten taitojen opettamisessa.
Family centered care of adult patients and learning of family centered care described by newly graduated nurses
The purpose of this study was to describe family centered care of adult patients and learning of family centered care described by newly graduated nurses. The research data were collected by interview of 13 newly graduated nurses. The informants were working in primary health care or specialized medical care for 12 15 months. The data was analyzed by inductive content analysis.
The newly graduated nurses described family centered care in their work by meanings of workload, exchanging information, internal support of the family and meanings of the family oriented cooperation relationship. The family centered care was described as a workload, when related to a suitable situation or time, the encounters with the family was affected by difficulties adding to the workload, the lack of family burdened the nurse or the family remained distant. Exchanging information between the family and the nurse meant supporting the family with information or the family as informants. The nurses described informing the family as their duty. The internal support of the family included patient centeredness, where the family was seen as unique and the support of the family important for the patient. The expectations of the family members directed the communication. The nurse s responsibility of the family as the target of nursing care was highlighted in the family oriented cooperation relationship. The nurse was an initiator towards families and felt responsible of the wellbeing of the family. The family was seen as an everyday part of nursing care, where the emphatic support of the family members is central. The cooperation between the nurse and family was described as mutual action.
The newly graduated nurses learned family centered care in their education, as humans and in working life. The learning of family centered care in education was enabled by theory facts, the curriculum, or by practical training. Individual teachers were described as meaningful sources of learning. The education produced a meaning for family centeredness in nursing care. The nurses described that learning as humans was enabled by individual qualities or experiences. The encounter experiences and evaluation of own actions supported learning. Learning family centered care in working life occurred in the work place procedures, gaining of skills, absorption of knowledge and also learning from other nurses.
The findings of this research add knowledge of family centered care of adults. The findings suggest that family centeredness is not self-evident. On the other hand a newly graduated nurse is able to engage in a cooperation relationship that supports the entire family. The findings can be utilized in development of teaching family centered care and family centered skills.
