Tiivistelmä
Tämä Pro gradu -opinnäytetyö koostuu kirjallisuuskatsauksesta sekä artikkelista, joka on otettu arvioitavaksi tieteelliseen lehteen (Tuomisto Sonja, Koivula Meeri & Joronen Katja: Perheen osallisuus sepelvaltimotautipotilaan kuntoutumisessa (POSEK) – mittarin kehittäminen ja arviointi).
Sydän- ja verisuonisairauksien hoito ja ehkäisy ovat merkittävä osa perusterveydenhuoltoa ja ne ovat kolmanneksi suurin työkyvyttömyyttä aiheuttava sairausryhmä. Verenkiertoelinten sairaudet ovat sekä naisten että miesten yleisin kuolinsyy. Pohjois-Eurooppalaisilla miehillä on suurempi riski kuolla sydäntautiin kuin Välimeren maissa asuvilla miehillä.
Perheenjäsenen sairastuminen on uhka perheen hyvinvoinnille. Perhe on usein tärkein tukija potilaalle ja perheiden tulisi saada asianmukaista tukea pystyäkseen olemaan tukena sairastuneelle perheenjäsenelle. Tuen puutteen on todettu olevan yhteydessä kuolleisuuteen sekä pitkittyneeseen sairaalassaoloaikaan.
Tämän kirjallisuuskatsauksen tiedonhaku on toteutettu lokakuussa 2013. Tiedonhaut kohdistuivat Cinahl-, Medline- ja Medic-tietokantoihin. Lisäksi tutkimuksia etsittiin käsinhaulla jo valittujen tutkimusten lähdeluetteloista. Aineisto koostuu kymmenestä pohjois-eurooppalaisesta tieteellisestä tutkimuksesta, jotka täyttivät sisäänottokriteerit. Aineisto analysoitiin sisällön erittelyn avulla.
Tulosten mukaan sydänpotilaat ja heidän perheenjäsenensä saivat hoitohenkilökunnalta emotionaalista, tiedollista, päätöksentekoon liittyvää sekä konkreettista tukea. Tuen saamisessa ilmeni kuitenkin puutteita erityisesti tiedollisen tuen osalta. Hoitohenkilökunnan käytöksellä ja asenteella oli vaikutusta potilaan ja hänen perheenjäsentensä turvallisuuden tunteen kokemiseen. Perhehoitotyön toteutumisen kannalta on tärkeää selvittää potilaiden ja heidän perheenjäsentensä kokemuksia saadusta tuesta, jolloin tuki-interventioiden kehittäminen ja kohdentaminen olisi helpompaa. Kirjallisuuskatsauksen tulokset antavat suuntaa siitä, millaiseksi sydänpotilaat ja heidän perheenjäsenensä kokevat saadun tuen.
In-hospital support for heart patients and their family members
This Master's thesis consists of a literature review and a scientific article that is being evaluated in a scientific journal (Tuomisto Sonja, Koivula Meeri & Joronen Katja: Development and evaluation of the Family Involvement in Coronary Artery Disease Patients Rehabilitation (POSEK)–scale).
Treatment and prevention of cardiovascular diseases are an important part of primary health care and they are the third largest group of diseases causing disability. Cardiovascular diseases are the most common cause of death of both women and men. Northern European men have a higher risk of dying from heart disease than men living in Mediterranean countries.
The illness of a family member is a threat to family well-being. The family is often the main source of support for the patient and family should receive appropriate support in order to support the sick family member. Lack of support has been found to be associated with mortality and prolonged hospitalization.
The information search of this literature review was implemented in October 2013. Information searches focused on Cinahl-, Medline- and Medic-databases. In addition, studies were searched by hand. The data consists of ten Northern European scientific studies that met the inclusion criteria. The data were analyzed by content analysis.
The results show that heart patients and their family members received emotional support, informational support, affirmation and concrete aid from the nursing staff. However, there were some deficiences. Especially informational support was inadequate. The behavior and attitude of nursing staff had an impact on the sense of security of heart patients and their family members. From the realization of Family Nursing it is important to find out the experiences of heart patients and their family members of the support received from nursing staff to support the development and allocation of interventions. The results of the literature review give an indication of how the heart patients and their family members experience the support received from nursing staff.
