Tiivistelmä
”Muiden silmissä vaan häirikkö..” ─tutkimuksen tarkoituksena oli selvittää, mitä hoidon ja avun saannin esteitä samanaikaista mielenterveys- ja päihdehäiriötä sairastavat kohtaavat. Kummankin sairauden yhteisesiintyminen on hyvin yleistä niin päihde- kuin mielenterveyspotilaidenkin kohdalla. Mielenterveys- ja päihdehäiriön yhteisesiintymistä kutsutaan komorbiditeetiksi, yleisemmin puhutaan kaksoisdiagnoosista.
Ihmisellä ilmenevä päihde- ja mielenterveyshäiriön yhtäaikainen esiintyminen vaikuttaa merkittävästi kummankin häiriön oirekuvaan, kulkuun, hoitoon ja niiden ennusteeseen. Päihteidenkäytön ja mielenterveyhäiriön yhteisesiintyminen altistaa monelle terveyttä ja sosiaalista hyvinvointia uhkaavalle tekijälle sekä lähes poikkeuksetta aina syrjäytymiskierteelle, joista seuraa monimuotoisia avun-, tuen- ja hoidontarpeita. Mielenterveys- ja päihdepalveluiden eriytyneisyys johtaa valitettavan usein kuitenkin tilanteeseen, jossa kaksoisdiagnoosin omaavat ihmiset jäävät vaille tarvitsemaansa apua ja hoitoa.
Tämän tutkimuksen mukaan kaksoisdiagnoosipotilaiden hoidon toteutuminen perustuu helposti sattumanvaraan ja yksilön omaan kykyyn ajaa omia asioitaan. Eettisesti ja perustuslaillisesti tarkasteltuna tilanne on kestämätön. Tutkimusaineiston perusteella hoidon ja avun saannin esteet muodostuvat palvelujärjestelmän epätarkoituksenmukaisesta rakenteesta ja yleisestä asenteellisuudesta. Palvelujärjestelmän rakenteesta aiheutuvat vaikeudet ilmenevät palveluiden epätarkoituksenmukaisuutena, tiedonvajeena sekä epätasa-arvoisuutena. Asenteellisuus hoidon ja avun saannin esteinä esiintyy tämän tutkimuksen mukaan kaksoisdiagnoosipotilaiden epäinhimillisenä, leimaavana ja syrjivänä kohteluna sekä piittaamattomuutena.
Aineisto tutkimukseen on kerätty fokusryhmähaastatteluin. Fokusryhmiä muodostui yhteensä viisi siten, että nais- ja miespotilaat, heidän omaisensa sekä julkisen ja kolmannen sektorin työntekijät muodostivat kukin oman ryhmänsä. Ryhmähaastattelut nauhoitettiin ja litteroitiin, jonka jälkeen aineisto analysoitiin sisällön analyysi-menetelmällä. Tutkielma on laadullinen ja aineistolähtöinen, induktiivinen luonteeltaan.

ABSTRACT
The aim of this study was to ascertain the barriers of care and help that people who suffer from both mental disorders and substance abuse problems meet. It is common that among people with mental or substance misuse problems suffer from both of the disorders at the same time.
The occurrence of both disorders is called comorbidity; proverbial name for a person with both disorders is dually diagnosed patient.
The occurrence of both mental disorders and substance abuse problems prejudice remarkably to the symptoms of both disorders, the progression, treatment and the prognosis of care. The appearance of both mental disorders and substance abuse problems exposes the patient for many health and social welfare threatening elements and almost without exceptions to isolation. These factors cause multiform needs of social and health care services. Unfortunately, in Finland the mental health and social services are organized separately─ this often leads to a situation where dually diagnosed patients might miss out on help and care.
According to this study the implementation of care for dually diagnosed patients is easily based on randomness and/or to the patients own ability to fight for the services. Ethically and constitutionally this situation is unbearable.
On the grounds of this research material the barriers for getting help and care consist from the inexpedient structure of the social and health care system and common biases towards the dually diagnosed people. The disadvantages consist of inexpedient of services, common lack of knowledge and inequality. Bias as an obstacle to getting help and care, appears ─according to this study as inhumanity, stigmatisation, nonchalance and discrimination.
The research material was collected by focus group interviews. There were five focus groups, gathered with the following manner: male patients, female patients, next of keens, employees of public sector and employees of the NGO:s. After the interviews of five different occasions the recorded and lettered material was analysed with content analysis. This study is qualitative and inductive by nature.
